Steps to Complete Your Fit Test

Register and Select Add-Ons
- Register for the fit test and choose any additional services you wish to include.

Complete the Medical Evaluation Form
- Accurately fill out the online medical evaluation form. (You will automatically receive an email with a keycode after completing registration.)

Access the Password-Protected Link
- Upon submitting your form, you will receive a secure, password-protected link via email. (If the email isn’t in your inbox, please check your spam folder.)

Review and Schedule Your Appointment
- After reviewing your submission, we will send you the medical evaluation clearance and a scheduling calendar via email.

Book Your Fit Test
- Use the provided calendar to schedule your fit test at a convenient time.

Prepare for Your Fit Test
- Bring the respirator you plan to use, and allocate 20–30 minutes to complete the test.

Receive Your Certification
- Once you pass the fit test, you will receive a laminated, wallet-sized certification card valid for one year. Please note: recertification will be required if your weight changes by 30 pounds or more within the certification period.

All records are retained for one year and are then securely destroyed in compliance with HIPAA standards. This includes shredding documents and other physical media to ensure the information is rendered unreadable and cannot be reconstructed.
